Summary:
Under the guidance and supervision of the Coordinator of Health Home Care Management Program (HHCMP) or designee, the Outreach / Engagement Specialist contacts individuals referred to the program to start the information and enrollment process for care management services. He/she also initiates community outreach activities to promote case management services in the community and identify individuals that need such services.

The employee must be resourceful and able to assess situations to determine if a crisis is involved and how to best proceed within the framework of the agency’s as well as the program’s policies and procedures. The employee will consult with supervisor if in doubt prior to proceeding.

Responsibilities
- Contacts individuals referred to HHCMP within 3 days of receiving the referral and within 24 hours when the individual referred is in the hospital.
- Initiates and prepares a plan for community outreach activities to promote case management services.
- Identifies individuals in the community in need of health care services, social services and case management services.
- Verifies that the referred individual meets program requirements including having an active Medicaid case.
- Competes all required paperwork with the referred or identified individuals to enroll them into the HHCMP.
- Maintains accurate and updated records on each individual related to the outreach, information and enrollment process.
- Ensures compliance with program policies and procedures as required by funders.
- Maintain individuals’ records using the Netsmart Care Management System and any other pertinent data collection system.
- Attends staff meetings and formal/informal training sessions in/outside the agency.
- Participates in community activities as representative of the program when assigned.
- Is available to work outside regularly scheduled work hours as needed.
- Any other duties as may be assigned by the immediate supervisor.

- Bilingual in Spanish and English.
- Demonstrated ability to work with other service providers.
- Basic knowledge of Microsoft Word.
- Able to provide own transportation and a valid good standing NYS driver’s license.
- An equivalent combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO
