Human Touch Home Health has an immediate need for a Physical Therapist (PT) to provide home health visits to patients in Northwest Washington, DC.
We have patients who need care now and are looking for a DC-licensed PT who can add visits to their current schedule or build a PRN caseload with our team.

What You'll Do
- Complete PT evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Assess home safety and fall risks
- Educate patients and caregivers
- Monitor patient progress and update treatment plans
- Complete progress assessments and discharge planning
- Coordinate with physicians, nurses, and other members of the care team
- Complete timely documentation using WellSky
Qualifications
- Active Washington, DC Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Dependable transportation to patient visits
- Strong clinical and communication skills
- Ability to work independently in patients' homes
- Home Health and OASIS Experience Preferred, but Not Required
- WellSky Experience is a Plus
