What You’ll Do
-
Perform comprehensive in-home patient assessments
-
Develop and implement individualized care plans
-
Provide skilled nursing care including wound care, medication management, and disease education
-
Complete accurate and timely OASIS documentation
-
Educate patients and caregivers on disease management and safety
-
Coordinate care with physicians and interdisciplinary team members
-
Monitor patient progress and report changes appropriately
What We’re Looking For
-
Active Pennsylvania Registered Nurse (RN) license
-
Minimum 1–2 years nursing experience (home health preferred)

OASIS experience preferred
-
EMR experience (WellSky/Kinnser or Homecare Homebase a plus)

Strong assessment and documentation skills
-
Valid driver’s license & reliable transportation
