- Process agency home health referrals by reaching out to clinicians in an accurate and timely manner.
- Work in collaboration with all members of the pod to ensure times Start of Care and Resumption of Care for all patients.
- Track and follow up with all patients who are pending start of care.
- Report all non-admit patients to the Intake Supervisor/Administrator.
- Serve as the liaison between the company and patients/caregivers.
- Interact with referral sources and the sales team regarding new referrals.
- Understand and can effectively explain all programs offered by the company.
- Ability to learn and retain medical terminology and protocols.
- Electronic filing
- All other duties assigned by supervisor.

- Associates Degree Preferred/Bachelor’s degree and/or work equivalent
- At least one (1) year of experience in home health care is preferred.
- One year of administrative support experience
- Familiar with medical terminology is preferred
