Position:
PRN Home Health Speech Therapist providing in-home speech therapy to adult and geriatric patients.
Responsibilities:

Perform in-home speech, language, cognitive, and swallowing evaluations

Develop and implement treatment plans per physician orders
-
Provide skilled speech therapy interventions
-
Educate patients and caregivers
-
Complete timely and accurate documentation
-
Collaborate with interdisciplinary care team
-
Maintain HIPAA and Medicare compliance
Qualifications:
-
Graduate of accredited Speech-Language Pathology program
-
Active Maryland Speech Therapy license
-
Minimum 2 years clinical experience (home health preferred)

Adult/geriatric experience
-
OASIS and EMR experience preferred (WellSky/Kinnser or HCHB)
-
Valid driver’s license and reliable transportation
