Position Overview:
We are seeking a dependable Licensed Practical Nurse (LPN) to provide in-home care to patients in the Falls Church area on a PRN basis.

What You’ll Do:
- Provide skilled nursing care in patients’ homes under RN supervision
- Administer medications and treatments as ordered
- Monitor patient conditions and report changes to the RN/physician
- Assist with wound care and other clinical procedures
- Educate patients and caregivers on care plans
- Maintain accurate and timely EMR documentation
What We’re Looking For:
- Licensed Practical Nurse (LPN) in Virginia (or eligible)
- Minimum of 1 year of nursing experience required
- Home health experience preferred, not required
- Strong clinical, communication, and organizational skills
- Ability to work independently and travel between patient homes
