The CARE Team Member - Case Manager provides direct case management services to individuals with severe and persistent mental illness/addiction and is responsible for assisting in developing, coordinating, and monitoring programming activities.

Job Knowledge and Comprehension:
- Provides specialized treatment, assistance, education, rehabilitation, and support services as part of a comprehensive team to C.A.R.E. team clients.
- Completes comprehensive assessments and evaluations necessary for establishing client eligibility for C.A.R.E. services.
- Participates as part of a comprehensive team in the treatment planning process and monitors consumer progress in meeting the goals and objective of the treatment plan.
- Assesses, monitors, locates, and coordinates necessary medical, social, psychiatric, financial, residential, vocational, and daily living needs and services for C.A.R.E. team clients.
- Provides emergency services including crisis assessment and intervention to C.A.R.E. team clients.
- Works cooperatively with other agencies, organizations, individuals, and community services to develop resources and ensure comprehensive treatment for C.A.R.E. team clients.
- Other duties as assigned consistent with non-xempt functions.
Physical Requirements:
Required to sit for long periods of time. Infrequent light physical effort required lifting up to 10 lbs. occasionally and 25 lbs. rarely. The employee frequently is required to sit and reach with hands and arms. Incumbent may be required to work extended periods of time at a computer monitor. Must be able to drive vehicle occasionally.
Qualifications:Associates degree required in social work, psychology or other related field
Prefer Bachelor’s degree in social work, psychology or other related field and two years of experience
Possession of a valid South Dakota Driver’s License
Ability to work as part of a team providing specialized services
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above.
