Working within the Harm Reduction Paradigm, the position of nurse practitioner will support the mission and goals of The Night Ministry (TNM) by providing health care, education, and prevention services across various outreach settings, including the Health Outreach Bus, Street Medicine, CTA, and youth programs. This role involves offering hospitality, basic care, health education and assessments, counseling, medical treatment, and referrals for further appropriate care to individuals experiencing homelessness and those without access to adequate health care, while improving and expanding TNM's health care capacity.

What You’ll Do:
- Accurate and timely data entry in external and internal databases, i.e., Electronic Health Records (EHR) and Efforts to Outcomes (ETO).
- Conduct basic physical assessments and provide appropriate treatment for acute and chronic conditions in pediatric, adult, and elderly populations.
- Provide Medication Assisted Treatment for Opioids and support clients in engaging and maintaining treatments for any/all substance use disorders.
- Ability to work closely with and support other staff and volunteers during the planning and provision of outreach services
- Offer referrals for health care, including updating and managing list of referrals.
- Maintain documentation of health-related activities and other services provided, as required by funding or accrediting sources.
- Manage storage and inventory of supplies.
- Participate in internal and external training as required.
- Able and willing to travel to and provide services at other outreach settings, such as shelters.
- Attend all department, agency and inter-agency meetings as required.
- According to The Night Ministry’s policies, this position is expected to carry a cell phone and is eligible for partial reimbursement of monthly cell phone bills.
- Complete other tasks as assigned.
What We’re Looking For:
- Must possess an M.S.N. degree and current Advanced Family or Adult Practice Nursing License.
- Possess DEA license or is willing to obtain one.
- Minimum 2 years’ nurse practitioner experience.
- Demonstrated experience in counseling patients/clients.
- Excellent interpersonal skills.
- Ability to react quickly in emergency situations; demonstrated “street smarts”.
- Physically able and willing to work in various settings, including outdoors; this position requires riding the Health Outreach Bus for Bus shifts and vans for Street Medicine and/or the CTA.
- Familiarity with referral sources.
- Current driver’s license.
- Proven capability to manage the organization’s website and all graphics and photography.
- Ability to work collaboratively in a team environment committed to fulfilling the mission, supporting each other, and having fun along the way.
- Must be available evenings and weekends.
