Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health status by taking vital signs such as blood pressure, pulse, temperature, and respiration rates
- Support clients with medication administration following care plans and HIPAA confidentiality standards
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
- Maintain a clean and safe environment through regular cleaning tasks and infection control practices
- Use specialized equipment such as Hoyer lifts or resident’s
for safe transfers and mobility assistance - Observe and document patient conditions, behaviors, and responses to treatments or activities for ongoing care planning
- Collaborate with nursing staff, social workers, and family members to ensure comprehensive patient care
- Assist with transportation to medical appointments or community activities in accordance with care plans
- Follow all safety protocols including CPR, First Aid procedures, and emergency response actions
Requirements
- Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly preferred
- Knowledge of Alzheimer’s care, dementia care, memory care, autism spectrum disorder support, and working with individuals with developmental disabilities or disabilities in general
- Certifications such as CPR and First Aid are required; additional training in HIPAA compliance is a plus
- Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina for long shifts involving standing or walking
- Strong observational skills for patient monitoring and reporting changes in condition accurately
- Excellent communication skills to interact effectively with clients, families, and healthcare teams
- Compassionate attitude with a commitment to respectful and dignified care delivery
- Flexibility to work various shifts including evenings or weekends if needed
