Overview
The Programs Coordinator will oversee management of HIV and HCV prevention and care coordination services to individuals participating in harm reduction services for care coordination and prevention contracts. This position oversees Ryan White Part A, Methamphetamine, and Hepatitis C harm reduction contracts. This position coordinates the provision of mental health and substance use counseling, health education, viral load suppression, support groups, PrEP and PEP coordination services, and other social services to members with substance use and/or other chronic health conditions. This position also oversees the Hepatitis C prevention and navigation services. This coordinator will monitor and assure the provision of targeted services to program participants. The program will assist in reducing emergency room visits, hospitalizations, and better coordination of services.
Contingent on credentials: Should this worker holding LCSW credential provide clinical supervision to qualifying mental health counseling staff as needed. This worker will also quality assure the delivery of mental health and supportive counseling interventions to program participants in harm reduction.

Responsibilities:
- Must have strong management experience with a team of workers to achieve program enrollment goals and efficient service activities.
- Determines eligibility for program participants in RWA and DOHMH prevention contracts.
- To generate resources for program staff including social services, professional development, and individualized improvement plans as needed.
- Assures the completion of initial assessment and quarterly reassessments on all assigned clients in a timely manner in-order to develop individualized treatment goals for each client.
- Assures consistent establishment of treatment goals and objectives for clients as part of the interdisciplinary team.
- Assess that clients’ needs for services are developed cooperatively with the client, family, and other providers as needed.
- Provides case management services including but not limited to weekly and monthly progress summaries, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site) and discharge planning.
- Able to conduct or coordinate the provision of Seeking Safety interventions (individual and group), Healthy Conversations, and other evidence-based interventions with clients.
- Facilitates harm reduction strategies support groups as needed.
- Provides weekly supervision to team members to ensure support and consistent delivery of services.
- Ensures timely submission of required clinical documentations to care organizations to avoid interruption of services.
- Coordinates smooth transition of program participants to other settings as per the care plan.
- Work collaboratively with team members to provide outreach and engagement too hard to reach clients.
- Assist clients to navigate the health insurance system including the AIDS Drug Assistance Program (ADAP).
- Aid through advocacy and psychoeducation to clients and family for entitlements and other community services.
- Understands and is sensitive to chronic medical conditions, including HIV, mental health and substance use, diabetes, asthma, hypertension, heart disease and hepatitis diseases.
- Consistently exhibit Housing Works core values of teamwork, advocacy, and customer service.
- Schedule weekly, biweekly, or monthly clinical supervision meetings with eligible mental health and counseling staff.
- Facilitate group clinical supervision as needed.
- Provide support to counseling interns as needed.
- Coordinate clinical support to intern students with graduate institutions as prescribed by higher learning.
- Approve practicum hours to students and licensed staff as per Office of Professions standards.
- Assist clients in navigation of internal and external medical systems
- Conduct annual quality assurance and quality improvement assessments and strategies to assure optimal performance.
- Conduct field work to members’ homes and other provider settings as needed to fulfill plan of care.
- Participate in regularly scheduled case conferences.
- Attends clinic’s morning huddles as needed.
