What are the responsibilities and job description for the 40 Hr RN Case Manager (#26-07; Posted 2/27/26) position at Houlton Regional Hospital?
Must be an RN with active Maine license. Primary duties will be social service responsibilities of admission assessment, discharge planning and coordinating continuum of care with outside facilities. This individual will also be responsible for assisting with emergency room referrals. Additional duties will include patient education on chronic disease, follow up phone calls and coordinating of medical care. The RN must be willing to learn the rules and regulations of the Utilization Review process. The ideal candidate must be self-motivated, have strong written and oral communication, be flexible and a team player.
