General Statement of Duties: The Admissions Support Nurse (RN) determines initial nursing care needs of hospice patients and enrolls patients in the hospice program. Provides nursing care to patients and families based on assessed needs reflected in the plan of care. Serves as a flexible clinical resource supporting multiple clinical teams to ensure timely admissions, patient visits, and continuity of care.
Level of Responsibility: Reports to Nurse Manager, Access Team
Status: Non- Exempt; Full-Time; Hourly
Pay Range: $37.25/hour - $46.00/hour
Schedule: Monday - Friday, 10:00 a.m. - 6:30 p.m., with two rotating weekend days per month.

Duties and Responsibilities:
- Enrolls patients in the hospice program and educates patients and families regarding hospice services, disease progression, symptom management, and end-of-life care.
- Performs nursing assessments to identify patient and family needs and develops appropriate plans of care.
- Consults with the Hospice Medical Director or designee regarding hospice eligibility, admission appropriateness, and covered medications.
- Completes hospice admissions, pre-admission assessments, urgent admissions, follow-up assessments, and related documentation in a timely manner.
- Provides nursing interventions, medication review, patient education, and symptom management consistent with the plan of care.
- Collaborates with physicians, pharmacies, care teams, patients, and families to ensure continuity of care and positive patient outcomes.
- Provides phone triage, patient assessments, late-day referral visits, and After Hours support as needed.
- Supports Access, Home Care, and Inpatient Facility (IPF) teams by completing comprehensive assessments, routine visits, admissions, and other patient care activities based on organizational needs.
- Maintains flexibility to work across service areas, responding to changing referral volumes, staffing needs, and patient care priorities.
- Participates in on-call coverage, staff development activities, and organizational initiatives while maintaining hospice competency, productivity standards, and compliance with agency policies.
