Registered Nurse (Jefferson County, WV)

Duties & Responsibilities

1. Conduct comprehensive assessments of patients' physical, psychosocial, and safety needs to develop and update individualized plans of care.
2. Coordinate and provide direct nursing care, admissions, symptom management, and patient visits in accordance with the established plan of care.
3. Collaborate with the interdisciplinary team, patients, and families to establish, revise, and maintain plans of care that support quality end-of-life care.
4. Educate and support patients, families, caregivers, and facility staff regarding hospice philosophy, disease progression, pain management, and symptom control.
5. Communicate patient status changes with physicians, obtain and implement orders, and coordinate medication and treatment updates as needed.
6. Establish and maintain positive working relationships with patients, families, physicians, community partners, and nursing facility staff to ensure continuity of care.
7. Document assessments, interventions, patient/family education, physician communications, and other required clinical information in a timely and accurate manner.
8. Assess, supervise, and coordinate hospice aide services, including development and ongoing evaluation of aide care plans.
9. Provide after-hours support, phone triage, crisis care, and on-call coverage as assigned, responding to patient and family needs in a timely manner.
10. Support care within nursing facilities by participating in care planning meetings, assisting with wound care management, identifying hospice-appropriate patients, complying with facility requirements, and serving as a hospice resource for facility staff.

Additional Responsibilities: Participate in staff meetings and educational programs, assist with inpatient facility staffing as needed, maintain organizational productivity standards, and perform other duties as assigned.
