Position Summary:
The Hospice of Baton Rouge is seeking a compassionate and experienced Full-Time Clinical Manager to oversee hospice nursing operations across homecare and the Inpatient Unit (IPU), providing leadership, supervision, and coordination of staff and daily operations.
The Clinical Manager ensures the delivery of high-quality patient care while maintaining compliance, operational efficiency, and a collaborative team environment.
Job Functions:

Provide daily leadership, guidance, and supervision to nursing staff, promoting strong teamwork, communication, and morale.

Conduct regular one-on-one and team meetings to evaluate performance, provide feedback, and support professional development.

Assist with recruiting, hiring, onboarding, and orientation of homecare and IPU nursing staff.

Assess clinical competencies and ensure care assignments align with staff skill levels and patient/family needs.

Implement performance improvement plans as needed to address individual or team development areas.

Mentor and support nursing staff, fostering a culture of continuous learning and accountability.

Ensure the delivery of high-quality, patient-centered hospice care in accordance with organizational standards and regulations.

Monitor patient care plans to ensure they are individualized and appropriate for each patient’s needs.

Address concerns from patients, families, and partner facilities in a timely and professional manner.

Review and analyze patient outcomes to identify trends, improvement opportunities, and best practices.

Support staffing, scheduling, and assignment management to ensure adequate coverage and balanced workloads across homecare and IPU teams.

Track and evaluate staff productivity, including visit frequency, time management, and caseload distribution.

Collaborate with leadership to establish and achieve productivity goals.

Address operational challenges and implement solutions to improve efficiency without compromising quality of care.

Oversee the accuracy, completeness, and timeliness of clinical documentation within patient medical records.

Conduct regular chart audits to identify and correct documentation inconsistencies or gaps.

Train and support staff on documentation standards and best practices.

Ensure compliance with all local, state, and federal regulations, as well as organizational policies and procedures.

Monitor and manage patient-related expenses while maintaining high standards of care.

Oversee the appropriate use of medical supplies, medications, and equipment to minimize waste and control costs.

Collaborate with finance and leadership teams to manage expenses such as overtime, travel, and supplies.

Review and approve time, mileage, and PTO requests for assigned staff.

Provide regular updates and reports on operational performance and expenses to leadership.

Assist in the development and implementation of educational programs and in-service training.

Ensure nursing staff maintain required certifications and continuing education.

Provide direct patient care support as needed, including admissions, informational visits, skilled nursing visits, and Service Intensity Add-On visits.

Participate in on-call rotation or continuous care as needed.

Support community education initiatives and organizational events.

Education

Associate’s or Bachelor of Science in Nursing (BSN preferred)
Certifications

Current Louisiana Registered Nurse (RN) license

Current CPR certification
Work Experience
- Minimum of three (3) years of nursing experience in acute care, home health, or hospice
- Minimum of two (2) years of hospice experience preferred
- Prior leadership or supervisory experience preferred
