Social Worker, LMSW , LCSW, LMHC - Part Time

Job Overview:

Horizon Family Medical is looking for a therapist/behavioral health clinician to join our vibrant imbedded behavioral health team for a part time position between our New Windsor and Goshen, NY offices.

At Horizon Family Medical Group, therapists are embedded in primary care practices to work collaboratively with physicians, patients, and family members to identify behavioral health needs and provide consultation, counseling, and referrals to needed resources. The position requires a professional who is experienced, warm, flexible, adaptable, and able to address a variety of patient needs of all age groups (mental health, substance use, caregiver support, social needs, chronic disease management, adjustment to a new diagnosis, etc.)

Days and Hours:

Monday - Friday 8:30am - 4:30pm - 20 to 25 hours within these days and times.

Job Duties:

• Conduct comprehensive assessments to identify clients' needs and develop individualized treatment plans.
• Collaborate with physicians, patients, and family members to identify behavioral health needs to provide holistic care
• Provide brief therapeutic interventions utilizing appropriate modalities,
• Utilize evidence-based practices to support clients in overcoming challenges related to mental health and emotional well-being.
• Provide skills training on disease self- management, improved coping, distress tolerance and stress reduction
• Provide consultation and coordination of care for patients with primary care staff
• Provide substance use/abuse evaluations, counseling and develop harm reduction strategies
• Provide referral to community resources based on individual needs
• Maintain accurate documentation of client progress and treatment outcomes in accordance with regulatory standards.
• Travel to the New Windsor, NY and Goshen, NY locations

Experience

• A background in behavioral health counseling or a related field is essential.
• Familiarity with crisis intervention techniques and tele-health practices is preferred.
• Knowledge of psychopharmacology as it relates to mental health treatment is beneficial.
• Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds

Qualifications:

• Licensed mental health professional (LMSW*, LCSW, psychologist-looking to gain clinical hour**, LMHC, or MHC working towards their clinical hours**)
• *Clinical supervision is available to all LMSWs working towards their clinical certifications.

• Outpatient or Inpatient clinical or mental health clinical experience (one year experience preferred)
• Required to travel to our New Windsor and Goshen, NY offices
