Home Instead is seeking an experienced Director of Patient Services (RN) to lead clinical operations in our Merrick office.
This is a hands-on leadership role for an RN who understands home care, thrives on accountability, and is confident managing both clinical quality and caregiver training.

- (2) years of home care experience
- Active New York State RN License
- Proven experience with PCA and/or ACD training programs
- Strong knowledge of NYS DOH and LHCSA regulations
- Ability to lead, train, and hold teams accountable
- Prior supervisory or DPS experience preferred
- Reliable vehicle to travel throughout the community and perform home visits and assessments
Schedule: Flexibility is a must
- Part-Time [3 Days Per Week]
- Paid [On-Call]
- Normal office hours are (8am-4:30pm) but there is the occasional scheduled evening or weekend visit that will need to be covered
What You’ll Do
Clinical Leadership
- On-site training and Supervision of the field staff (PCAs, HHAs, RNs)
- Oversee all patient care services and clinical operations
- Ensure full compliance with NYS DOH and LHCSA regulations
- Review and manage plans of care and patient cases
- Maintain documentation, audits, and quality assurance standards
- Supervise and support caregivers and nursing staff
- Comfortable with wound care, and / or med administration
Field Oversight
- Conduct initial assessments and re-assessments
- Perform post-hospitalization visits
- Ensure patients are receiving appropriate, high-quality care
- Participate in on-call rotation (paid)
Training Leadership (Required)
- Lead PCA training programs (NYS compliant)
- Facilitate Advanced Caregiver (ACD) training
- Ensure all training meets DOH standards and documentation requirements
- Evaluate caregiver competency and maintain training records
