Responsibilities Include:
- Visit clients in the privacy of their residence in designated geographic territories.
- Schedule and preform monthly supervisory visits
- Schedule and preform Quarterly Evaluations
- Complete timely and accurate clinical notes including addressing patient's progress.
- Evaluate the client status and the skills of workers while the staff member is present.
- Keeps documentation of all communication with clients, family members and support staff. .
- Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
- Counsels and educates the patient and family in meeting healthcare and related needs.
- Write 485/ Plan of Care for patients when reauthorizations are due for patients.
- Other projects/duties as assigned.
- Home Care experience preferred
- Excellent verbal and written communication skills.
- Highly motivated, organizational, leadership and relationship building skills
- Ability to work in a very detailed, fast paced environment
- Currently holds a RN license in PA
- Valid state driver’s license and car insurance
