Compensation: $150-$200 per visit

What You'll Be DoingBe The Clinical Quarterback
- Perform comprehensive Start of Care assessments
- Complete accurate and timely OASIS documentation
- Identify risks, needs, and opportunities early
- Create individualized care plans that guide the interdisciplinary team
- Educate patients and families with compassion and confidence
Shape The Patient Experience
- Help patients transition safely from hospital to home
- Coordinate care with physicians, therapists, and case managers
- Build trust from the very first visit
- Serve as a resource and advocate for patients and families
What You BringRequired
- Active Minnesota RN license
- Strong Start of Care and OASIS experience
- Reliable transportation and valid driver's license
- Excellent assessment and communication skills
Preferred
- Home health or community-based nursing experience
- Passion for patient education and relationship-based care
