JOB SUMMARY
LifeSpring In Home Care of Eastern PA is a seeking a team-oriented, seasoned registered nurse to join our vibrant and growing home health care agency as a Registered Nurse (RN) case manager. This person will oversee all aspects of the patients plan of care with those involved with the patients care.
LifeSpring In Home Care of Eastern PA is a family owned, locally operated and nationally supported home health care agency. Our home health care is patient first focused that furnishes an efficient and cost-effective alternative to hospitalization or other institutional care, allowing patients to comfortably remain in their place of residence. We provide patients with skilled nursing care; physical, occupational and speech therapy; and behavioral health care, among other services. Our team is comprised of highly trained home health care professionals dedicated to going above and beyond to ensure the care and comfort of our patients and their families, while simultaneously addressing individual needs and providing high quality, patient-focused and loving care.
We currently serve patients throughout the Lehigh Valley, primarily in Lehigh and Northampton Counties.
Responsibilities and Duties
- Provide and direct the provision of home care through plan of care development and implementation of the care plan.
- Communicate with physicians, other RN's, LPNs, Therapists and Other Medical Professionals.
- Assist field staff in completion of the care plan and supplemental orders, and coordination of care.
- Complete the documentation of skilled need and achievement of outcomes based on agency policies.
- Ensure that services have been completed as ordered for timely billing.
Qualifications and Skills
- RN Certification in the Commonwealth of Pennsylvania
- Willingness to work as part of a team (On Call based on rotational basis. On Call is paid in addition to regular salary)
Job Type: Full-time
Pay: $65,000.00 - $85,000.00 per year
