At Home Health Care, Inc., we're hiring a Physical Therapist to provide one-on-one care throughout the Forest Lake, Chisago Lakes, Hugo, North Branch, and surrounding North/Northeast Metro communities. You'll still manage a productive caseload, but your workday is built around individual patient visits rather than a waiting room full of people competing for your attention.

When you arrive at a patient's home, that's your patient. You're evaluating their mobility, watching how they move, understanding what's limiting them, providing skilled treatment, educating caregivers, and determining what needs to happen next.

There's no second patient warming up while you're treating the first. No one waiting at the front desk because the schedule is running behind. You can focus on the person you're there to treat and then move on to your next visit.

A Territory That Makes More Sense Than a 19-City List

You'll serve patients throughout the North and Northeast Metro, with scheduling organized around geography and patient needs. Our primary service hubs include Forest Lake & Hugo, Chisago Lakes & Lindstrom, North Branch & Wyoming, and Lino Lakes & surrounding communities.

Our broader service area also includes nearby communities such as Center City, Centerville, Columbus, Harris, Marine on St. Croix, Scandia, Shafer, Spring Lake, Stacy, and Taylors Falls. You won't be expected to bounce from one end of the entire territory to the other with every visit; the goal is to create an efficient field schedule based on where patients are located.

Treat the Patient in Front of You

Your patients may be recovering from surgery, hospitalization, illness, falls, neurological conditions, or significant functional decline. Some need to regain strength and endurance, while others need gait training, transfer training, balance work, equipment recommendations, or caregiver education.

The common denominator is that your attention isn't divided among several patients at once. You can assess what's happening, provide the appropriate intervention, measure progress, and determine what comes next.

What You'll Be Doing

• Complete comprehensive in-home Physical Therapy evaluations
• Develop individualized plans of care and functional goals
• Provide skilled interventions addressing strength, balance, gait, transfers, mobility, and safety
• Assess fall risks and barriers to independence
• Recommend equipment and appropriate safety modifications
• Educate patients, families, and caregivers
• Monitor progress and adjust treatment plans when clinically appropriate
• Coordinate with PTAs, nurses, physicians, OTs, and other clinicians
• Complete accurate and timely clinical documentation
• Maintain a productive field-based patient schedule

You Don't Need Home Health Experience to Be a Strong Candidate

Great home health PTs can come from Outpatient Orthopedics, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, or Home Health itself. What matters is having strong clinical judgment, organization, communication skills, and the confidence to work independently with your patients.

What You Bring

• Active Minnesota Physical Therapist license
• PT or DPT
• Valid driver's license and reliable transportation
• Strong clinical assessment and decision-making skills
• Ability to independently manage a productive field schedule
• Excellent patient and caregiver communication
• Strong organization and documentation skills
• Home health experience preferred
• OASIS experience preferred
