Home Health Care, Inc. is hiring an experienced Physical Therapist (PT / DPT) for a role focused specifically on Start of Care admissions throughout the Twin Cities Metro and surrounding communities.

This role puts the evaluation side of Physical Therapy at the center of your work.

You'll meet new patients, complete comprehensive assessments, establish functional baselines, identify mobility and safety concerns, determine therapy needs, and develop the initial plan of care.

Then the ongoing clinical team takes that plan forward.

You get to specialize in evaluating where patients are, determining where they need to go, and establishing how we're going to help them get there.

A great Start of Care PT doesn't simply run through a checklist.

You're assessing how multiple factors fit together.

Strength. Balance. Gait. Transfers. Endurance. Pain. Fall history. Equipment. Home setup. Cognition. Caregiver availability. Medical complexity. Prior level of function.

Then you have to answer the bigger question:

What does this patient actually need from home health?

For a PT who enjoys evaluations and clinical reasoning, that's the interesting part.

What You'll Be Doing

• Complete comprehensive Start of Care Physical Therapy evaluations
• Assess strength, gait, balance, endurance, transfers, mobility, and fall risk
• Establish functional baselines and identify primary barriers to independence
• Evaluate patients within their actual home environments
• Complete OASIS and admission documentation accurately and on time
• Develop individualized plans of care based on clinical findings, physician orders, and patient goals
• Establish appropriate therapy frequency, goals, and interventions
• Identify needs for assistive devices and durable medical equipment
• Provide initial patient and caregiver education
• Identify when additional disciplines or resources should be involved
• Collaborate with OT, SLP, nursing, physicians, social work, and other members of the care team
• Complete timely electronic documentation and maintain regulatory compliance

This Is a Different Kind of PT Workday

Instead of walking into your fifth treatment visit with a patient whose plan you've already established, you're continually encountering something new.

A post-operative orthopedic patient may need a relatively straightforward pathway forward. Your next admission could involve someone with multiple chronic conditions, repeated falls, limited caregiver support, and a home environment that changes your entire recommendation.

Each admission requires you to assess, think, decide, and build.

That's why we're looking for a PT who genuinely enjoys the evaluation side of clinical practice.

What You Bring

• Current Minnesota Physical Therapist license in good standing
• Current CPR certification
• Valid driver's license and reliable transportation
• Ability to travel throughout an approximately 75-mile service radius
• Strong comprehensive assessment and clinical reasoning skills
• Excellent documentation and organizational abilities
• Confidence making independent clinical decisions
• Strong patient, caregiver, and interdisciplinary communication skills
• Ability to independently manage a field-based schedule

Experience That Will Help You Stand Out

• Previous home health experience
• OASIS experience
• Start of Care or admission experience
• Knowledge of Medicare home health requirements
• Strong evaluation and plan-of-care development experience
• Experience with orthopedic, neurological, cardiac, pulmonary, geriatric, or medically complex populations

Optional Weekends Give You Additional Earning Potential

Weekday Start of Care visits pay $100 per visit plus mileage reimbursement.

If you want to increase your earnings, optional weekend admissions are available at a higher $120 per-visit rate.

Don't want to work the weekend? Don't pick up the visits.

And holidays are completely off.
