Home Health Care, Inc. is looking for an experienced Occupational Therapist (OTR/L) to put those higher-level assessment skills at the center of their role by focusing on Start of Care admissions.

Rather than primarily carrying an ongoing treatment caseload, you'll focus on the point where assessment, clinical reasoning, documentation, care planning, and interdisciplinary coordination all come together.

You'll meet patients at the beginning of their home health episode, establish the clinical baseline, identify their needs, and help determine the direction of care.

You're Not Just Completing the Assessment. You're Establishing the Baseline.

Every clinician who sees that patient afterward needs an accurate picture of where they're starting.

What can the patient safely do today? Where do they need assistance? What has changed since hospitalization? How is cognition affecting function? What is the caregiver realistically capable of providing? Are there environmental barriers? Which disciplines should be involved?

Your Start of Care assessment helps answer those questions and gives the interdisciplinary team something critical:

What You'll Be Doing

• Complete comprehensive Start of Care Occupational Therapy evaluations
• Perform detailed assessments of ADLs, cognition, functional mobility, upper-extremity function, caregiver support, and home safety
• Complete OASIS and admission documentation accurately and within required timeframes
• Establish an accurate functional and clinical baseline
• Develop individualized plans of care based on assessment findings, physician orders, and patient goals
• Identify safety concerns, fall risks, and barriers to independence
• Determine appropriate equipment and environmental recommendations
• Provide initial patient and caregiver education
• Coordinate care with nursing, PT, SLP, social work, physicians, and other disciplines
• Complete timely electronic documentation and maintain regulatory compliance
• Travel throughout the Twin Cities Metro and surrounding service area based on referral needs

This Role Requires More Than a Strong OT

We're looking for someone who understands home health.

You should be comfortable with the realities of walking into a new patient's home without knowing exactly what you'll encounter. You need to understand how to gather a large amount of clinical information efficiently, document it accurately, recognize what requires immediate attention, and communicate a clear plan to the rest of the team.

What You Bring

• Current Minnesota Occupational Therapist (OTR/L) license
• Current CPR certification
• Valid driver's license and reliable transportation
• Ability to travel throughout an approximately 75-mile service radius
• Strong clinical assessment and decision-making abilities
• Excellent documentation and organizational skills
• Confidence working independently in patient homes
• Strong interdisciplinary communication skills

Experience That Will Help You Stand Out

• Previous home health experience
• Strong OASIS knowledge
• Start of Care or admission experience
• Knowledge of Medicare home health requirements
• Experience developing plans of care
• Experience with medically complex, orthopedic, neurological, or geriatric patients

There's Still Room to Increase Your Earnings

Your regular weekday Start of Care visits pay $100 per visit plus mileage reimbursement.

If you'd like to add to your earnings, optional weekend Start of Care visits may also be available at a bonused $120 per visit. Weekend admissions aren't the centerpiece of the role, but they give you another opportunity when you want it.
