🩺 Position Summary
We’re growing rapidly and looking for part-time Physical Therapists (PTs) to join our dedicated team!
You’ll provide skilled, compassionate therapy services to home health patients, teach family members proper care techniques, and collaborate closely with the care team and physicians.
🔹 Key Responsibilities
✅ Evaluate patients within 72 hours of receiving a physician referral
✅ Develop and implement individualized Plans of Care
✅ Provide therapy services and supervise PTAs and HHAs
✅ Communicate effectively with physicians and the interdisciplinary team
✅ Maintain accurate, timely documentation in EMR
✅ Evaluate treatment effectiveness and update care plans as needed
✅ Participate in interdisciplinary team meetings and continuing education
✅ Maintain patient confidentiality per HIPAA standards
🎓 Qualifications
✔ Active Physical Therapist (PT) license
✔ Minimum 1 year of PT experience (Home Health/OASIS preferred)
✔ WellSky (Kinnser) EMR experience a plus
✔ Physical (within 6 months of first patient contact)
✔ Current TB test (within last 12 months)
✔ Current CPR certification
✔ Valid driver’s license & reliable transportation
✔ Must be available to attend orientation at our Willowbrook office
✔ Available Monday–Friday, 8 AM–5 PM
