Innovate at Home is seeking a compassionate, skilled Physical Therapist Assistant (PTA) to join our growing home-based care team. This is a great opportunity for a PTA who loves autonomy, flexible scheduling, and meaningful one-on-one time with patients.

What You’ll Do

• Provide high-quality therapy services under the direction of the Physical Therapist

• Support patients in achieving their mobility and functional goals through individualized treatment plans

• Deliver care in patient homes with professionalism, empathy, and strong clinical judgment

• Document visits thoroughly and timely in accordance with regulatory and company standards

• Partner with the care team to ensure exceptional outcomes for each patient

• Educate patients and families on exercises, safety, and care plans

What We’re Looking For

• Current PTA license in the state of Nebraska (or eligibility)

• Experience in home health, outpatient, or rehab setting preferred

• Strong communication skills and ability to work independently

• A passion for providing patient-centered care in a home environment

• Reliable transportation and willingness to travel locally
