Job Title: Home Health Physical Therapist Assistant
Job Responsibilities
- Provide skilled physical therapy to clients in their homes, including therapeutic interventions and goals, home exercise program, etc.
- Coordinate with other members of the care team to ensure comprehensive and integrated care for clients
- Educate clients and their families on proper self-care techniques and health maintenance strategies
- Monitor and document client progress and communicate any changes or concerns to the physician
- Act as an advocate for clients and ensure their needs are met in a timely and respectful manner
- Adhere to all company policies, procedures, and regulatory requirements
- Active PTA license in the state of Nebraska
- Minimum of 2 years of physical therapy assistant experience, preferably in home health or a similar setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively as part of a team
- Compassionate and patient-centered approach to care
