What You’ll Do
- Support residents and families through admissions, transitions, and ongoing care planning
- Conduct psychosocial assessments and develop individualized care plans
- Advocate for resident needs and ensure emotional and social well-being
- Facilitate care conferences and communicate updates with families and interdisciplinary teams
- Assist with discharge planning and coordination of services
- Ensure compliance with all federal, state, and facility regulations
- Provide support during difficult conversations and life transitions with empathy and professionalism
- Bachelor’s degree in Social Work or related field required
- Social Services Designee certification (or willingness to obtain) preferred
- Experience in long-term care, post-acute, or senior living preferred
- Strong communication and interpersonal skills
- Ability to navigate sensitive situations with compassion and professionalism
- Highly organized with strong follow-through
