What You’ll Do

• Support residents and families through admissions, transitions, and ongoing care planning
• Conduct psychosocial assessments and develop individualized care plans
• Advocate for resident needs and ensure emotional and social well-being
• Facilitate care conferences and communicate updates with families and interdisciplinary teams
• Assist with discharge planning and coordination of services
• Ensure compliance with all federal, state, and facility regulations
• Provide support during difficult conversations and life transitions with empathy and professionalism
  
  

What We’re Looking For

• Bachelor’s degree in Social Work or related field required
• Social Services Designee certification (or willingness to obtain) preferred
• Experience in long-term care, post-acute, or senior living preferred
• Strong communication and interpersonal skills
• Ability to navigate sensitive situations with compassion and professionalism
• Highly organized with strong follow-through
