What You’ll Do
- Provide primary and preventative care to geriatric patients in home and community-based settings
- Manage chronic conditions with a focus on quality of life and functional outcomes
- Perform assessments, diagnoses, and treatment planning
- Collaborate with interdisciplinary teams, caregivers, and families
- Support transitions of care and reduce unnecessary hospitalizations
- Document visits and care plans in a timely and accurate manner
- Autonomy & Flexibility – Manage your schedule and patient flow
- Smaller Patient Panels – More time for meaningful care
- Mobile Practice Model – No traditional clinic setting
- Mission-Driven Work – Direct impact on aging adults in our community
- Supportive Team Environment – Clinical and operational support provided
- Licensed Nurse Practitioner (APRN) in Nebraska (or ability to obtain)
- Board certified (AANP or ANCC)
- Experience in geriatrics, primary care, hospice, or home health preferred
- Strong clinical judgment and ability to work independently
- Compassionate, relationship-focused approach to care
- Valid driver’s license and willingness to travel locally
