Duties and Responsibilities Crisis intervention; Maintain assigned
caseload; Providing direct care to patients/residents ? including
assessments, individual counseling, discharge and aftercare planning,
treatment plans and updates, weekly progress notes, daily groups
facilitation, group note completion, and completion of bio-psychosocial
assessments; Making referrals when appropriate; Responsible for each
assigned patients/residents? medical records. All records are to be
professional, complete and thorough with entries made in a timely
manner; Responsible connecting patients/residents with aftercare
services prior to discharge and aiding the resident and his/her family
access services when appropriate; Communicating/involving family in
treatment at patients/residents? request; Attend and participate in all
unit, facility meetings, case conferences, Treatment Team meetings, and
mandatory trainings, as directed by the Clinical Supervisor /Director;
Responsible for continued education in the field of substance use and
related courses. Always utilize standards precautions; Demonstrates
behaviors that recognize the rights of patients as defined by the
patient?s rights; Residential Services Adherence, to insurance
regulations, including requesting extensions after 90 days of treatment
if needed; Supporting patients in crisis through verbal de-escalation,
1:1 monitoring, or crisis/safety planning, as requested; Participation
in program initiatives, quality projects, and program support services.
