Title: Wellness Director (LVN)
Status: Full-Time, Exempt
Reports To: RCFE Administrator / Executive Director
Pay Range: $85k-$95k annually

You will lead with both heart and discipline. That means keeping every resident's health and wellness at the center of decisions, staying ahead of state regulations, and creating a workplace where caregivers and medication aides want to stay. Families and surveyors will see a well‑run clinical operation – and you will be the reason.
WHAT YOU BRING TO THE FACILITY

THE FOUNDATION
-
Current, unencumbered Licensed Vocational Nurse (LVN) issued by the State.

License in good standing with no active probation or substantiated violations.

Graduate of an accredited nursing program.

THE JOURNEY
-
Minimum 2 years of clinical experience in assisted living, memory care, or skilled nursing.

Minimum 1 year of supervisory experience in a senior living or healthcare setting.

Proven track record of maintaining resident health outcomes and survey readiness.

Experience managing a clinical department budget, staffing, and medication administration systems.

THE CRAFT

Deep fluency in applicable state regulations for assisted living / RCFE (including medication management, resident assessments, and care planning).

Ability to implement and oversee health and wellness programs, including chronic disease management and fall prevention.

Strong clinical assessment skills – you know when a resident is declining and when to escalate.

Staff development: train, mentor, and retain caregivers and medication aides.

Proficiency with electronic health records and clinical documentation systems.

WHAT YOU WILL BE DOING

Oversee all clinical services, including resident assessments, care plans, and medication management.

Lead and mentor caregivers, medication aides, and other wellness staff.

Manage state surveys; implement Plans of Correction as needed.

Collaborate with the RCFE Administrator on budgets, staffing, and quality improvement initiatives.

Monitor resident health metrics (falls, weight loss, medication errors, hospitalizations) and drive improvement.

Coordinate with outside providers, including physicians, hospice, and home health agencies.

Ensure compliance with all state and federal regulations related to assisted living clinical care.

Conduct new resident move‑in assessments and ongoing health evaluations.
