Salary: $68,640 - 72,800

Your Mission:

The Social Services Director is responsible for planning, organizing, implementing, and directing the Social Services Department in accordance with federal, state, and local regulations, as well as facility policies and procedures. This role ensures the psychosocial and emotional needs of patients are met, advocates for patient rights, and serves as a vital liaison between patients, families, staff, and community resources. The ideal candidate will possess strong clinical skills, empathy, and leadership ability.

Key Responsibilities:

Clinical and patient-Centered Services:

  • Conduct comprehensive psychosocial assessments upon admission and as needed.
  • Develop and implement individualized care plans addressing patients’ psychosocial, emotional, and cultural needs.
  • Provide counseling, support, and crisis intervention to patients and families.
  • Facilitate adjustment to placement, coping with illness, and end-of-life planning.
  • Advocate for patients’ rights, dignity, and autonomy.

Care Coordination and Discharge Planning:

  • Collaborate with interdisciplinary teams (nursing, therapy, dietary, etc.) to ensure integrated care.
  • Coordinate discharge planning, including arranging home services, transfers, or alternative placements.
  • Ensure safe and timely discharges with appropriate follow-up resources.
  • Maintain knowledge of community resources and make referrals as needed.

Regulatory and Compliance:

  • Ensure compliance with federal (CMS), state, and local regulations related to social services.
  • Participate in facility surveys and implement plans of correction as needed.
  • Maintain accurate, confidential documentation in patient records.
  • Stay updated on changes in healthcare laws and social work best practices.

Leadership and Department Management:

  • Supervise and mentor social services staff (if applicable).
  • Develop and manage departmental budgets and resources.
  • Provide training to facility staff on psychosocial topics, patient rights, and abuse prevention.
  • Participate in quality assurance programs and committees.

Family and Community Engagement:

  • Act as primary contact for families regarding psychosocial concerns.
  • Organize family meetings and support groups as appropriate.
  • Build and maintain relationships with community agencies, hospitals, and service providers.

Ideal Candidate Profile:

  • Bachelor’s Degree in Social Work (BSW) preferred.
  • Minimum of [2] years of experience in a healthcare setting, preferably in long-term care or skilled nursing required.
  • Prior supervisory experience preferred.
