What you will be doing
The Care Manager is responsible for planning, organizing, supervising, and evaluating the delivery of care and nursing services within assisted living and memory care settings. The Care Manager manages a resident caseload, ensures regulatory compliance, implements community policies, and upholds mandated infection-control procedures. This role requires the Care Manager to be proficient in all clinical competency areas, including documentation, direct resident care, medication administration and management, patient rights, safety, and infection control.
The Care Manager collaborates with physicians, pharmacists, and clinical providers to meet residents’ healthcare needs and completes scheduled Level of Care evaluations to ensure appropriate care delivery. The Care Manager also monitors medication effectiveness and adverse reactions, provides training and education to Med Techs and Care Assistants, and participates in the on-call rotation. In addition, the Care Manager offers leadership, guidance, and support to the care staff to maintain high-quality, consistent resident care.

- Management experience required.
- Experience as a Care Assistant and/or Med Tech, required.
- Previous experience assisting with coordination of resident service plans, preferred
- Med Tech Certification, preferred
- CNA Certification, preferred
- Current CPR certification
