Now Hiring: Social Services Director

As the Social Services Director, you will play a vital role in supporting the emotional, social, and psychosocial well-being of our residents. You will collaborate with residents, families, and the interdisciplinary team to ensure each resident receives the resources, support, and advocacy needed to thrive.

Position Responsibilities

  • Complete accurate and timely Social Services documentation, including psychosocial assessments, MDS sections, care plans, progress notes, and other required documentation in accordance with state and federal regulations.

  • Support residents and families with emotional, social, and adjustment needs throughout their stay.

  • Participate in interdisciplinary care planning by identifying resident needs and contributing to individualized care plans.

  • Maintain thorough documentation reflecting resident progress, interventions, and response to care plans.

  • Provide referrals to community resources and support services as needed while maintaining appropriate records.

  • Assist with discharge planning and help residents and families prepare for successful transitions.

  • Serve as a resource for residents, families, staff, and community members regarding services, resources, and resident rights.

  • Coordinate and facilitate monthly Family Council meetings.

  • Support resident transitions, including room changes and adjustment to the long-term care environment.

  • Encourage resident and family participation in care plan meetings and promote resident-centered decision-making.

  • Maintain communication with residents, legal representatives, and approved family members regarding resident needs, care, and services.

  • Collaborate with the Activities Department and other team members to support residents who may have limited family involvement or additional needs.

  • Develop and maintain positive relationships with administration, nursing staff, physicians, consultants, residents, families, volunteers, and community organizations.

  • Partner with mental health providers to identify psychosocial needs, coordinate referrals, and ensure appropriate services are provided.

  • Assist with community education related to aging, resident rights, long-term care services, and available resources.

Qualifications

  • Bachelor’s degree in Social Work or a related human services field such as sociology, psychology, rehabilitation counseling, or special education.

  • Minimum of one year of supervised social work experience in a healthcare setting working directly with individuals.

  • Experience in skilled nursing, long-term care, or healthcare social services preferred.

  • Strong written and verbal communication skills.

  • Ability to maintain confidentiality and demonstrate professionalism at all times.

  • Excellent interpersonal skills with the ability to build trust and communicate effectively with residents, families, and healthcare professionals.

  • Strong organizational skills and the ability to manage multiple priorities in a fast-paced environment.

  • Team-oriented mindset with a commitment to providing exceptional customer service and resident care.
