Join Henderson Behavioral Health as a Coordinator and make a difference by bridging the gap between medical and behavioral health services. This position is responsible for the management and administrative operations of our Primary Care Program, HILL. They maintain supervisory responsibility over staff, coordinate care, produce reports, communicates with other HBH managers and staff in order to ensure collaborative and coordinated care for individuals served. Registered Nurse preferred, with at least 5 years' experience in working in a family primary care environment, serving adults and children. At least 2 years in a supervisory capacity and demonstrates the ability to lead a team. Knowledge of medical techniques such as administrating medications and injectables, monitory medical processes and general patient care
What You'll Do:
- Coordinate care by integrating behavioral health and primary care services.
- Schedule appointments, manage referrals, and track progress to ensure seamless service delivery.
- Follow up within 24 hours of emergency room visits or hospital discharges, in person or via telehealth.
- Connect clients to community-based services inside and outside the organization to meet their needs.
What You'll Need:
- Bachelor's degree in Behavioral Health Services, Human Services, or a related field (Master's preferred).
- A valid Florida driver's license and auto insurance.
