Job Responsibilities:
- Evaluate and assess residents’ functional abilities and needs
- Develop and implement individualized treatment plans
- Assist residents in improving daily living and self-care skills
- Collaborate with interdisciplinary team members to ensure comprehensive care
- Monitor and document resident progress and adjust treatment plans as needed
- Educate residents, families, and staff on therapy techniques and safety practices
- Maintain accurate and timely documentation in compliance with facility and regulatory guidelines
- Participate in care planning meetings and required in-service trainings
- Active Occupational Therapist (OT) license in California (required)
- Previous experience in a skilled nursing facility preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
