Responsibilities
- Evaluate and assess patients to develop individualized treatment plans
- Provide physical therapy services to patients recovering from injury, illness, or surgery
- Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care
- Monitor patient progress and adjust treatment plans as necessary
- Educate patients, families, and caregivers on therapy techniques and home exercises
- Maintain accurate and timely documentation in accordance with state, federal, and facility guidelines
- Ensure compliance with all regulatory and facility standards
- Contribute to team meetings and care planning for residents
- Graduate of an accredited Physical Therapy program
- Current and valid Physical Therapist license in the State of California
- CPR certification (current and valid)
- Experience in a skilled nursing or long-term care setting preferred, but not required
- Strong communication and documentation skills
- Ability to motivate and encourage patients in a compassionate, respectful manner
