Home Health RN Case Manager

Contract Length: 13 Weeks
Openings: 1
Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call: May be required based on business needs
Overtime: Possible (must be pre-approved)

Compensation & Work Details:

• Productivity: 30 points per week
• Mileage Reimbursement: $0.45/mile
• Territory: Baltimore City & Baltimore County
• • Potential coverage in Anne Arundel and Howard County if needed

Requirements:

• Active, unrestricted RN license in Maryland
• BLS/CPR certification required
• Valid driver’s license & car insurance
• OASIS experience required
• Wound care experience required
• HomeCare HomeBase (HCHB): Preferred

What We’re Looking For:

Seeking a dedicated, professional RN with strong communication skills and a commitment to timely, accurate documentation.

Primary Responsibilities:

• Conduct comprehensive assessments for high-risk patients
• Develop and implement individualized care plans in collaboration with physicians and care teams
• Continuously reassess and adjust care to ensure cost-effective, appropriate treatment
• Coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders
• Arrange non-medical support (transportation, housing, etc.) to ensure compliance with care plans
• Engage specialty resources to improve patient outcomes
• Maintain detailed documentation of clinical, functional, and financial outcomes
• Promote health education and preventative care
• Prevent and respond to adverse events to minimize patient risk
• Act as a patient advocate while ensuring confidentiality and compliance

Qualifications:

• Associate’s or Bachelor’s Degree in Nursing
• Active RN license (Maryland)
• Strong clinical judgment and ability to analyze complex medical information
• Case Management certification preferred
