Registered Nurse (RN) Care Manager
Location: Covering Chatham, Liberty, Bryan, and Effingham Counties (GA)
Schedule: Monday–Friday, 8:30 AM – 5:00 PM
Productivity Requirement: 30 units per 40-hour week
Mileage Reimbursement: $0.43 per mile
Overview
Seeking a compassionate and experienced Registered Nurse Care Manager to support high-risk members through comprehensive assessment, care coordination, and ongoing case management. This role focuses on improving health outcomes, ensuring appropriate levels of care, and connecting members with both medical and non-medical resources.
Primary Responsibilities
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Identify and reassess cost-effective and appropriate levels of care
- Coordinate care transitions and ensure continuity across settings
- Arrange non-medical support services (housing, transportation, etc.) to support treatment plan compliance
- Partner with physicians, social workers, discharge planners, and claims professionals to ensure smooth transitions of care
- Engage specialty resources as needed to achieve optimal patient outcomes
- Maintain detailed documentation of clinical, functional, and financial outcomes
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient events when possible and intervene quickly when necessary
- Serve as a patient advocate, ensuring privacy and confidentiality at all times
Qualifications
- Active, unrestricted Georgia RN license required
- Associate’s or Bachelor’s degree in Nursing (or related field)
- Case Management certification preferred
- Strong ability to analyze complex medical information and make sound clinical decisions
- Proficiency with OASIS documentation and accuracy preferred
- Experience using Homecare Homebase (HCHB) preferred
