Registered Nurse – Home Health
Schedule: Monday–Friday, 8:00 AM–4:30 PM EST
On-Call: Every 3 weeks — $25/day and $50/weekend day/holiday
Coverage Area: Shelby County, TN
We are seeking a Registered Nurse (RN) to provide comprehensive, patient-centered care in a home health setting. The RN will assess patients, implement and evaluate care plans, coordinate with healthcare providers, and support health maintenance and disease prevention.
Responsibilities
- Perform patient assessments and develop, implement, and evaluate individualized care plans.
- Provide skilled nursing care and case management in the home setting.
- Complete required clinical documentation, including OASIS and nursing notes.
- Monitor patient conditions and communicate changes to physicians and other healthcare providers.
- Coordinate medications, physician orders, appointments, and follow-up care.
- Collaborate with nurses, physicians, and other healthcare professionals.
- Supervise LPNs and support staff as needed.
- Provide health and safety education to patients and caregivers.
- Maintain compliance with infection control, medication management, and patient safety standards.
Requirements
- Active Tennessee RN license; compact licenses accepted.
- Home Health experience required.
- OASIS and HCHB experience required.
- Wound care competency preferred; training available if needed.
- Current BLS certification.
- T-Step TB test.
- Strong clinical, communication, and organizational skills.
Salary : $46 - $51
