Position Overview

We are seeking a knowledgeable and patient-focused Clinical Pharmacist to join our healthcare team. In this role, you will ensure the safe and effective use of medications while collaborating with physicians, nurses, and other healthcare professionals to deliver exceptional patient care. The ideal candidate is detail-oriented, clinically driven, and committed to improving patient outcomes through medication management and education.

Key Responsibilities

• Accurately review, verify, prepare, and dispense prescription medications in accordance with state and federal regulations.
• Monitor patient medication compliance, therapeutic effectiveness, and potential overuse or misuse.
• Evaluate prescriptions for accuracy, appropriateness, dosage, contraindications, allergies, and drug interactions.
• Counsel patients on prescription and over-the-counter medications, including proper use, storage, side effects, and potential interactions.
• Educate patients on the use of medical devices, including blood pressure monitors, glucose meters, and other self-management equipment.
• Compound medications for internal and external use as required.
• Prepare sterile medications and IV solutions using established aseptic techniques and safety standards.
• Maintain accurate and confidential patient records while ensuring compliance with pharmacy regulations and HIPAA requirements.
• Monitor and maintain pharmaceutical inventory, ensuring medications are stored appropriately and available when needed.
• Collaborate with physicians, nurses, and other healthcare professionals to identify and resolve medication-related issues.
• Recommend alternative therapies or dosage adjustments when clinically appropriate.
• Provide preventative health services such as smoking cessation counseling, blood pressure screenings, and cholesterol management.
• Supervise and mentor pharmacy technicians, interns, and support staff while ensuring compliance with pharmacy policies and procedures.
• Ensure compliance with all federal, state, and organizational pharmacy regulations and quality standards.

Qualifications

• Doctor of Pharmacy (PharmD) degree required.
• Active, unrestricted State Pharmacist License required.
• Strong knowledge of pharmacology, medication therapy management, and pharmacy regulations.
• Experience preparing sterile compounds and administering pharmacy best practices preferred.
• Excellent communication, clinical judgment, and patient counseling skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.
• Strong attention to detail and commitment to patient safety.
