Hospice Registered Nurse (RN) – Home Health | Wisconsin


We're seeking a compassionate Registered Nurse (RN) to provide in-home hospice care throughout Fond du Lac, Dodge, and Columbia Counties, with additional opportunities in Jackson, Waterford, and Sheboygan.


Schedule

  • Monday–Friday, 8:00 AM–5:00 PM
  • No weekends or on-call
  • Approximately 5 patient visits per day
  • Mileage reimbursement: $0.53/mile


Responsibilities

  • Provide skilled hospice nursing care in patients' homes
  • Perform comprehensive assessments and develop care plans
  • Deliver wound care, pain management, and symptom management
  • Educate patients and families on end-of-life care
  • Coordinate care with physicians and the interdisciplinary hospice team
  • Complete accurate EMR documentation


Requirements

  • Active Wisconsin RN license
  • Current CPR/BLS certification
  • Wound care experience
  • Valid driver's license, reliable vehicle, and auto insurance


Preferred

  • Hospice experience
  • HomeCare HomeBase (HCHB) experience preferred (MatrixCare or other EMR experience accepted)
  • Case management or home health experience
