Advanced Practice Provider

Brief Description

Our Advanced Practice Providers (Nurse Practitioners and Physician Assistants) are essential leaders within the care team, delivering high-quality, patient-centered care in a value-based environment. We prioritize quality over volume, with a target of 15–18 patients per day, allowing you to spend meaningful time with patients, focus on outcomes, and practice at the top of your license. Key responsibilities include:

• Provide comprehensive, patient-centered primary care aligned with a value-based care model
• Conduct patient evaluations, develop diagnoses, and implement evidence-based treatment plans
• Collaborate closely with physicians and multidisciplinary teams to ensure continuity and coordination of care
• Participate in quality improvement initiatives and support practice performance metrics
• Educate patients on preventive care, chronic disease management, and overall wellness
• Maintain accurate, timely documentation in the electronic health record (EHR)
• Ensure compliance with all regulatory standards, privacy requirements, and HealthyU policies and protocols
• Support the implementation of new technologies, clinical programs, and care delivery models
• Contribute to a positive, team-oriented clinical environment
• Perform additional duties as assigned
  
  

Requirements

• Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
• Active, unrestricted state license (must be active by start date)
• Current BLS certification
• Active DEA registration
• Minimum of 1 year of primary care clinical experience
• Experience providing longitudinal, patient-centered care
• Knowledge of value-based care, population health, and quality improvement practices
• Strong collaboration skills within a multidisciplinary care team
• Excellent communication and interpersonal skills
• Self-motivated with a strong commitment to clinical quality and patient outcomes
• Ability to provide two professional references
• Bilingual (Spanish) strongly preferred
• Experience with Athena EMR preferred
