Key responsibilities include:
- Provide comprehensive, patient-centered primary care aligned with a value-based care model
- Conduct patient evaluations, develop diagnoses, and implement evidence-based treatment plans
- Collaborate closely with physicians and multidisciplinary teams to ensure continuity and coordination of care
- Participate in quality improvement initiatives and support practice performance metrics
- Educate patients on preventive care, chronic disease management, and overall wellness
- Maintain accurate, timely documentation in the electronic health record (EHR)
- Ensure compliance with all regulatory standards, privacy requirements, and HealthyU policies and protocols
- Support the implementation of new technologies, clinical programs, and care delivery models
- Contribute to a positive, team-oriented clinical environment
- Perform additional duties as assigned
- Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
- Active, unrestricted state license (must be active by start date)
- Current BLS certification
- Active DEA registration
- Minimum of 1 year of primary care clinical experience
- Experience providing longitudinal, patient-centered care
- Knowledge of value-based care, population health, and quality improvement practices
- Strong collaboration skills within a multidisciplinary care team
- Excellent communication and interpersonal skills
- Self-motivated with a strong commitment to clinical quality and patient outcomes
- Ability to provide two professional references
- Bilingual (Spanish) strongly preferred
- Experience with Athena EMR preferred
