Local Territory Only: Your patients are in Santa Clara County / South Bay. You will never be asked to drive to San Francisco or the East Bay.

Annual Salary: Starts at $135,000 per year (Increases based on experience & Case Management skills)

Performance Bonus: Earn an additional up to 25% of your annual wage based on quality-of-care metrics.

Responsibilities

Provide skilled nursing care in accordance with the patient's individual Plan of Care.

Collaborate with the Interdisciplinary Team (PT, OT, ST, MSW) to ensure holistic outcomes.

Supervise Licensed Vocational Nurses (LVNs) when assigned.

Evaluate patient progress and report changes to the Case Manager or Director of Nursing.

Complete documentation in a timely manner using our AI-powered charting software.
Requirements

Current California RN License.

1 year of nursing experience (Home Health preferred).

Valid Driver's License and reliable transportation.

Compassionate, self-driven, and ready to work independently.
