As the "Air Traffic Controller" of our office, you ensure that patients transitioning from hospitals to their homes receive care without delay. This is a fast-paced role that requires someone who can juggle logistics while maintaining a compassionate touch.

To provide world-class care, our office remains responsive seven days a week.

Your Key Responsibilities

Manage Referrals: Process incoming patient referrals from hospitals and providers with speed and accuracy.

Coordinate Care: Work directly with patients and clinical teams to assign and schedule home visits.

Problem Solve: Proactively troubleshoot scheduling conflicts to ensure continuity of care.

Data Management: Maintain flawless records in our software systems.
What You Bring to the Table

Experience: Minimum 1 year in healthcare intake/scheduling (Home Health or Hospice experience is a major plus!).
