- This is a dedicated, full-time position.
- Your core hours will be a regular Monday through Friday shift.
- Participation in a rotating weekend coverage schedule is essential to this role to ensure our patients always have support.

- Work directly with patients and clinical teams to assign and schedule home visits.
- Proactively troubleshoot scheduling conflicts to ensure continuity of care.
- Maintain flawless records in our software systems and healthcare CRMs.
- Minimum 1 year of experience in healthcare intake or scheduling (Home Health or Hospice experience is a major plus!).
- Ability to speak professionally and empathetically with doctors, families, and clinicians.
- You thrive in a "moving parts" environment and can pivot priorities quickly.
- Comfortable navigating healthcare CRMs, digital scheduling platforms, and medical terminology.
