Responsibilities
- Conduct psychosocial assessments and develop discharge plans to support safe transitions to home or alternative care settings
- Coordinate care across inpatient teams and with community resources (home health, rehab, long-term care) to optimize outcomes
- Document assessments, care plans, and interventions in the electronic charting system with accuracy and timeliness
- Facilitate referrals to appropriate services and coordinate follow-up arrangements post-discharge
- Collaborate with the interdisciplinary team (including PT, interpretation services, social services, pharmacy, transportation) to support patient and family goals
Requirements
- Licensure: Current Washington social work license types accepted (LICSW, LSWAIC, LASW, LSWAA). RN applicants must hold WA RN license or Compact RN where applicable; verify status with client prior submittal
- Certifications: BLS (required)
- Experience: Minimum 1 year of clinical experience with acute care, discharge planning and care coordination; diverse case management experience preferred (Med-Surg, prolonged LOS, ED)
- Education: Bachelor's degree in Social Work or Nursing; MSW preferred for the social work pathway
- Skills: EPIC charting experience; ability to work in a high-volume acute care setting; willingness to float across units and two campuses
