Responsibilities
- Provide clerical support for Case Management, including authorizations, prior authorizations, post-acute referrals, and delivering Medicaid notices
- Coordinate post-acute referrals (rehab, long-term care, home care) and assist with benefits eligibility and discharge planning
- Document care coordination in EPIC and communicate with internal/external teams; coordinate DME orders and access to community services
- Maintain a typical caseload of 18–24 patients (up to 30 on weekends) with interdisciplinary support from PT, Interpretation Services, Social Services, Pharmacy, and Transportation
- Ensure compliance with regulatory and privacy requirements; support accurate charting and care coordination across the care continuum
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day)
Requirements
- Epic charting experience (required)
- Minimum 1 year of related clinical or case management experience (first-timers accepted)
- Washington license not required for this role
- Ability to work in a hospital-based case management setting with adult and geriatric populations
