Responsibilities
- Timely communication to physicians, families, and other care team members about significant changes in resident condition
- Complete applicable resident assessments and ensure accurate documentation
- Administer medications following the 5 Rights, and monitor for adverse effects
- Perform wound care and related assessments as needed
- Assist CNAs with answering call lights and resident needs
- Monitor residents for changes in condition and respond appropriately
- Act as floor leader when needed by assessing and directing situations
- Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator
- Maintain thorough, accurate resident records and charting; ensure data integrity
- Perform other duties as assigned to support resident care and safety

Requirements
- Wisconsin Registered Nursing license, active and in good standing
- Two-year degree in nursing required; four-year degree in nursing preferred
- Current CPR/BLS certification required
- 1 years of related work experience preferred
- An equivalent combination of education, training, and experience will be considered
- Ability to work in a long-term care setting and adapt to a dynamic team environment
