Responsibilities
- Administer prescribed medications and document administration according to physician orders and facility policy
- Monitor residents’ conditions, assess for changes, and report deviations to the charge nurse promptly
- Assist with activities of daily living (ADLs) and provide supportive care to residents to maintain comfort and dignity
- Obtain and interpret routine vital signs, respond to call lights, and escalate concerns as needed
- Maintain accurate nursing notes, care plans, and documentation within the electronic or paper charting system
- Collaborate with the interdisciplinary team to coordinate care plans, safety interventions, and discharge planning when applicable

Requirements
- Active Licensed Practical Nurse license in New Hampshire (or eligibility for licensure in NH)
- Prior experience in a long-term care or skilled nursing setting preferred
- Ability to work the evenings shift and adapt to a rotation as needed
- High school diploma or equivalent; completion of an approved practical nurse program
- Ability to meet travel assignment requirements and pass standard pre-employment screenings
