Responsibilities
- Provide professional nursing care to residents in the long-term care unit, including medication administration and ongoing assessments
- Document care plans, progress notes, and responses to treatment in the electronic medical record
- Conduct routine and focused assessments, monitor vital signs, and recognize changes in patient condition
- Collaborate with the interdisciplinary care team to develop, implement, and update care plans
- Support admissions, transfers, and discharges as needed; communicate with physicians and nursing leadership
- Ensure compliance with safety, infection control, and resident advocacy standards
Requirements
- Active Hawaii RN license
- Current BLS certification
- Minimum of one year of recent Long-Term Care or skilled nursing experience
- Experience with electronic medical records
- Ability to work the assigned schedule, including weekends and holidays as needed
